What does a teleradiologist do all day?
A teleradiologist's shift is built around a worklist. Studies arrive from client facilities by DICOM, already validated and prioritized, and the reading platform assigns them by priority tier and, in subspecialty practices, by modality and body part. The radiologist opens the study alongside prior imaging and the clinical history, interprets, dictates, and signs. STAT studies interrupt everything else; a stroke-protocol head CT does not wait behind routine outpatient volume.
Around the interpretation itself sit four recurring tasks that buyers should know exist, because they are where service quality lives:
Critical findings calls. A suspected critical result generates a phone call to the ordering clinician, documented on the report with time and recipient. On a busy overnight shift these calls are constant, and a service's callback discipline is measurable: ask what fraction of critical findings are phoned within 15 minutes of sign-off.
Clinical callbacks. Referring physicians call with questions about signed reports. A well-run practice routes that call to the interpreting radiologist or a covering subspecialist within minutes.
Addenda. New information arrives after signature: a missed prior surfaces, a clinician adds history. The radiologist issues an addendum against the original report. Addendum rates are a quality signal; consistently high ones suggest studies are being read without adequate clinical context.
Peer review. In practices with a real quality program, a fraction of the shift is spent blind-reviewing colleagues' cases. The American College of Radiology's guidance on teleradiology practice is explicit that teleradiologists should participate in formal peer review, and equally explicit that ghost reading, where one physician interprets and another signs, is unacceptable.
What a teleradiologist does not do is procedures. Biopsies, drainages, and fluoroscopy stay with onsite physicians, which is why facilities with procedural volume run hybrid coverage even when all diagnostic reads are remote.
The training behind the signature
The pipeline that produces a teleradiologist is the same one that produces every diagnostic radiologist in the United States, and it is long:

Board certification comes on top. The American Board of Radiology certifies diagnostic radiologists through a qualifying exam taken during residency and a certifying exam after it, and maintains certification through ongoing assessment. A minority of US radiologists certify through the American Osteopathic Board of Radiology instead; both are legitimate. Counting undergraduate study, the shortest path to a board-certified radiologist is 13 years, and a fellowship-trained subspecialist has spent 14 or 15.
For a buyer, two distinctions inside this uniform pipeline are worth money. The first is fellowship training: a neuroradiology fellowship changes what its holder sees on a subtle brain MRI, and which study types gain most from a subspecialty match is mappable. The second is current certification status, which is public and takes two minutes to check. Board-certified is a verifiable claim; "highly experienced" is not.
Licensing is the invisible half of the job
An onsite radiologist needs one state license. A teleradiologist needs one for every state their client facilities sit in, because licensure follows the patient's location at the time of imaging. A reader covering clients in twelve states holds twelve active licenses, each with its own renewal cycle, fees, and continuing education requirements. Teleradiology practices employ licensing teams for exactly this reason, and a reader's licensing footprint effectively determines which client studies can ever reach their worklist.
The Interstate Medical Licensure Compact has shortened the acquisition side: 44 states plus Washington DC and Guam participate as of 2026 per the IMLC Commission, and qualifying physicians can get expedited licenses in member states in weeks. What the compact did not create is a single national license, so the per-state verification burden on buyers is unchanged: for each state you operate in, the readers on your account need an active license you can look up on the state board's public site.
Hospitals carry a second layer: every remote reader must be credentialed and privileged at the facility, which CMS allows to run by proxy through the vendor under a written agreement. How does a teleradiologist differ from an onsite radiologist?
Not in qualification, as covered above. The real differences are operational, and honesty requires saying that some favor the onsite model.
Where onsite wins. An onsite radiologist attends tumor board, walks to the ED to look at a patient, knows referring physicians by name, and builds the kind of informal consult relationship a worklist cannot reproduce. They do procedures. And they see your facility's full case mix over years, which builds context no remote reader accumulates from a share of your volume.
Where the teleradiologist model wins. Subspecialty match: no single onsite radiologist is fellowship-trained in everything, and a panel routing by body part puts a neuroradiologist on the brain MRI and a pediatric radiologist on the child's abdominal CT at 3 a.m. Coverage: the worklist never takes a vacation, and volume spikes distribute across a panel instead of stacking on one person. And the night shift itself, which deserves its own paragraph.
Overnight reading is where staffing structure becomes a quality variable. A study published in the Journal of the American College of Radiology measured radiologists before and after overnight shifts and found diagnostic accuracy dropped from 0.926 to 0.806 after a night of continuous reading, with fatigue worsening on every measured dimension (Hanna et al., PubMed). An onsite radiologist covering nights is usually rotating on after daytime duties. A teleradiology practice can schedule dedicated night shifts with rest built in, so the person reading your 3 a.m. head CT is mid-shift and rested. Generalist or subspecialist: what kind of reader opens your study?
Teleradiology panels are not interchangeable, and the biggest structural difference between them is routing. A generalist operation assigns studies to the next available radiologist. A subspecialty operation routes by modality and body part before interpretation begins, so the study type determines the reader.
For high-volume plain films, the difference is modest; any board-certified radiologist reads a wrist X-ray competently. The gap opens on advanced imaging. Complex neuro, MSK, cardiac, oncologic, and pediatric studies read by a matching fellowship-trained subspecialist generate fewer addenda, fewer clinical callbacks, and fewer discrepancies flagged in peer review, which is why second-opinion subspecialty review changes management in a meaningful share of complex cases. The output you are actually buying: the report
Everything above describes the person; what lands in your EMR is a document, and a teleradiologist's skill is legible in it. Four markers separate a report worth its price from one that generates work downstream.
It answers the clinical question. The order said "rule out appendicitis"; the impression should address appendicitis explicitly, whatever else the study shows. Reports that describe findings without engaging the question force the clinician to interpret the interpretation.
The impression commits. Hedged impressions that list five possibilities without ranking them push the diagnostic burden back onto the referring provider. Good readers commit where the images support commitment and say plainly what would resolve the remaining uncertainty.
Follow-up recommendations track published guidelines. Incidental findings should carry recommendations consistent with ACR incidental findings guidance, with the relevant criteria named. Freelance follow-up intervals generate unnecessary downstream imaging, and your referrers notice.
Comparisons name the prior. "Compared to CT of March 12" tells you the reader actually opened it. "No significant interval change" with no prior named sometimes means no prior was available, which is a workflow failure worth catching early.
The clock matters alongside the content. - State licenses. Check each name against your state medical board's public lookup. Active, unrestricted, current.
- Board certification. Check each name against the certifying board's public verification. Note lapsed certifications; they happen.
- Fellowship claims. These live in the credential file, with the training institution named. Spot-check one or two.
- Malpractice coverage. Confirm the vendor's policy covers remote interpretation in your state, with limits your counsel considers adequate.
